Rates and Billings: 

Private Initial Assessment & Treatment: $85

Private Treatment: $80

MSP Rate: $20

**Cancellation Fee if less than 24 hours notice is given is:  $25.00. 

Direct Billing

We do offer direct billing to various extended health providers. 

Availability of this service is dependent on your carrier and your plan. Please note that all fees are the responsibility of the patient. If your insurance providers direct billing portal is not working on the day of your appointment, or the claim does not go through, you will pay the full amount of your visit up front. We will provide you with a receipt which you can then submit to your insurer for processing and reimbursement.

In order to bill the above providers we will need : 

1.     Your insurance card with policy number and member ID.

2.     A doctors referral, if required by your provider. 

3.     A valid piece of government-issued photo ID.  

4.     Full name and DOB of the primary member.
